Prenatal Providers

The Ohio GDM Postpartum Care Learning Collaborative developed several resources to aid providers in the treatment and care plan for women diagnosed with GDM. In consultation with nationally-recognized subject matter experts, along with feedback from participating providers across Ohio, a clinical toolkit and consumer resources have been developed for providers and support staff.

Provider Toolkit includes:

Screening and diagnostic criteria

Care coordination strategies and resources

Health and wellness educational messaging for consumers

Best practice flowchart for an office-based setting

Shared decision making guides

Consumer Resources (English and Spanish) include:

Impact of GDM on mother and baby

Recommended lifestyle changes

Dietary guidelines and education for maintaining recommended blood sugar

Benefits and impact of breastfeeding on mom and baby

The importance of receipt of an OGTT Screen by 12 weeks postpartum

Uncontrolled gestational diabetes during pregnancy can lead to high birth weight (>10), jaundice, preterm birth, or other conditions in the baby. Additionally, moms that had GDM are likely to experience a reoccurrence in future pregnancies or develop Type 2 Diabetes. In response to this growing problem, we have developed key information for both providers and consumers on GDM and Type 2 Diabetes. Please click on the images below for more information.
